Following a temporary discharge, PMG provides your facility with a copy of the readmissions most current orders to help your facility with a smooth transition back into the facility. New Admission Let the nurse’s focus on the resident; we will focus on the medications. The admission process can be as stressful for the nursing staff as it is for the new admission. PMG will automatically print MAR’s for all new admissions and deliver them with the medications same day. Person Centered CareOur experienced team of LNHA’s, RN’s, and Pharmacists are able to help your facility transition to toward an individual model of care. Person Centered Care is an approach to care that is becoming very popular in the State of Ohio and around the country. This philosophical approach to care is “centered” on the individual to meet their own specific needs and requests. There are many different areas of daily living that can be modified from more of a group or institutional approach to an individual model. As a facility determines their goals in achieving this model of care, they determine which one they wish to do first, such as dining, staff scheduling, and medication administration to name a few. Pharmacy Management Group became a part of this process with two of our customers, Eliza Jennings Home and The Renaissance when they wanted to adopt a medication administration model centered on the residents living at their facilities. The group determined that the delivery of medications within these two facilities would be based upon activities throughout a day instead of times of a day. A few examples of this are “upon arising” and “with meals”. The “pre-initiation” stage of this model is a lengthy process that involves a continual discussion among a very diverse group of people. This group of people will need to review all medication information, such as drug/food interactions and indications, dosing intervals, manufacturer’s indications, medication delivery and storage. The group needs to understand that changes will occur frequently as the population changes and the needs change. If the individual is always at the center of every discussion then the model of care will be achieved. Adaptable, flexible, customer focused. We made the proactive decision to include multiple Electronic Charting System options as a part of our service package. We feel that we should be able to adapt to systems that our customers choose, not chose for them. PMG is currently working with multiple systems and continually evaluating other vendors. These options include multi-system charting options and eMAR (electronic medication administration records) only options.
